How to enrol your student

Enrolment Information

Enrolment Documentation to be Provided: CHECK LIST FOR APPLICATION

Parents are asked to complete the Application For Enrolment form and return it to Rockingham Senior High School. With this form we ask that you include:

  • A copy of student’s last school report
  • A copy of student’s birth certificate
  • Immunization details (to ensure the immunization record presented to the school is up to date, parents are to obtain a current copy of the Immunization History Statement, obtained by:
    • telephoning ACIR on 1800 653 809 (free call)
    • emailing acir@humanservices.gov.au
    • visiting their local Department of Human Services Centre.
  • Proof of address (e.g. a copy of a utilities account or rental agreement)
  • Copies of Family Court or any other court orders (if applicable)
  • Information relating to suspensions or exclusions
  • Information relating to disability

If your child was not born in Australia, you must provide evidence of:

  • Passport or travel documents
  • Current visa subclass and previous visa subclass (if applicable)

If your child is a temporary visa holder, you must also provide:

  • Confirmation of enrolment or evidence of any permission to transfer provided by Education and Training International (ETI) at eti@dtwd.wa.gov.au. (if holding an International full fee student visa, sub class 571); or
  • Evidence of the visa for which the student has applied if the student holds a bridging visa.

This information is required for your application to be processed for stage 2 enrolment.
